Eligibility Requirements
We serve those who need it most. Here's who qualifies for our free services:
Uninsured or Underinsured
You have no health insurance or your coverage doesn't meet your needs
Income Below 200% FPL
Your household income is at or below 200% of the federal poverty level
Madison Area Resident
You live in Madison or the surrounding Dane County area

What to Expect at Your First Visit
Check In
Arrive 15 minutes early to complete paperwork. Bring any ID and proof of income if available (not required).
Meet Your Care Team
You'll meet with a culturally competent provider who will listen to your concerns without judgment.
Health Assessment
We'll do a thorough health check including blood pressure, glucose screening, and discuss any symptoms or concerns.
Create a Plan Together
Your provider will work with you to create a care plan that fits your life and addresses your health goals.
Connect to Resources
We'll help you access medications, specialists, insurance enrollment, and other community resources.
